PFSP Specialty Pharmacy provides custom referral forms in a PDF format for its New York physicians and providers. The forms can be filled out online, printed, and then faxed to the pharmacy. Please fill out the appropriate form for your patient and fax it back to PFSP at 866-286-4126.
